Position Overview
The Director of Case Management is responsible for the leadership, coordination, and daily oversight of the Case Management Department. This role ensures compliance with organizational policies and regulatory standards while driving efficient care coordination, discharge planning, and optimal clinical resource utilization.
The ideal candidate is an experienced leader with strong communication, presentation, and team-building capabilities. This role requires an individual who excels in dynamic environments, can manage multiple priorities, and effectively collaborates with internal and external stakeholders.
Key Responsibilities
- Direct and evaluate departmental operations, including staffing, onboarding, staff competencies, and case management workflows to achieve quality and performance goals.
- Allocate resources to meet productivity expectations and ensure adequate staffing.
- Oversee core Case Management functions and daily practice operations.
- Ensure compliance and quality in care coordination and discharge processes, including documentation standards.
- Collaborate with organizational leadership, including CFO and Division Case Management leadership, to meet performance benchmarks.
- Monitor departmental performance using data analysis and implement lasting process improvements.
- Ensure full compliance with policies and standard operating procedures.
- Build and maintain strong working relationships with clinical and administrative leadership (CMO, CNO, legal, ethics & compliance, ancillary departments).
- Serve as a subject-matter expert for Case Management and lead departmental change initiatives.
- Perform additional duties as assigned.

Qualifications
- Experience:
- 3+ years of recent acute hospital Case Management experience (within the last year required)
- 2+ years of leadership experience
- Education:
- Bachelor’s degree required
- Master’s degree in Nursing, Health Administration, or Business Administration preferred
- Licensure:
- RN, LCSW, or LMSW required
