Registered Nurse Case Manager (Hospice)

Location: Southwestern Virginia

Schedule: 40 hours/week - Monday - Friday On Call

Status: Full-time

What You’ll Do

  • Lead the Team: Act as the clinical lead for the interdisciplinary team, coordinating closely with physicians and families.
  • Holistic Care: Develop, implement, and document personalized patient care plans.
  • Advocacy: Ensure continuity of care and monitor cases for compliance and quality.
  • Mentorship: Supervise hospice aides and LPNs to maintain high standards of patient service.
  • Response: Participate in on-call responsibilities to ensure our families are never alone.


Qualifications

  • Education: Graduate of an accredited School of Nursing (Diploma, Associate, or BSN).
  • Licensure: Current State RN License and CPR certification.
  • Experience: 2+ years of clinical RN experience preferred (Home Health or Hospice experience is a plus).
  • Mobility: Must have a valid driver’s license, reliable transportation, and auto insurance.
  • Skills: Strong communication, problem-solving, and the physical ability to lift/transfer patients and equipment.
