Hospice Community Liaison – Hospital Focus

Compensation: $80,000-$150,000 +

Position Summary:

We are seeking a results-driven Hospice Community Liaison with a strong hospital focus to build and manage acute care referral relationships and drive hospice admissions. This field-based role centers on hospital rounding, case management engagement, and discharge partnership development.

Key Responsibilities

• Develop referral relationships within hospitals and acute care systems
• Conduct daily or scheduled hospital rounding
• Build partnerships with case management and discharge teams
• Provide hospice education and eligibility guidance
• Respond rapidly to referral opportunities
• Facilitate smooth hospital-to-hospice transitions
• Track referral activity and conversion outcomes
• Support bedside consult coordination when appropriate

Qualifications

• 3+ years hospice, home health, or post-acute liaison experience
• Strong hospital referral background REQUIRED
• Demonstrated census or referral growth results
• Knowledge of hospice eligibility and benefit structure
• Excellent clinical communication skills
• Self-directed and field-driven

Preferred

• Existing hospital relationships
• Hospice or palliative care sales experience
• Experience presenting to case management teams

Compensation

• Base + commission structure
• Mileage reimbursement
