Registered Nurse – Case Manager (Full-Time, Permanent)

Day Shift: 8:00 AM – 4:30 PM (includes 1 weekend per 4-week schedule)

Evening Shift: 12:00 PM – 8:30 PM (includes 1 weekend per 4-week schedule)
💼 Position Overview:
We are seeking a highly skilled and compassionate RN Case Manager to join a dynamic care coordination team in an acute care setting. In this role, you will help patients navigate their hospital stay while collaborating with interdisciplinary teams to ensure safe, cost-effective, and patient-centered care.
🩺 Key Responsibilities:
- Conduct comprehensive clinical and psychosocial assessments
- Create individualized, outcome-driven care plans
- Coordinate with physicians, nurses, and support staff to ensure appropriate level of care
- Lead discharge planning and connect patients with community resources
- Advocate for patients and families, ensuring clear communication and education
- Evaluate medical necessity of admissions and continued stays
- Monitor patient progress, document interventions, and contribute to performance improvement initiatives
- Maintain compliance with regulatory standards and best practices
✅ Qualifications:

Current RN license in the state of Florida or compact license (must obtain FL license within 90 days)

Associate Degree in Nursing required; Bachelor's Degree preferred
- Minimum 3 years of recent acute care RN experience, ideally in Med/Surg, Telemetry, ICU, or ED
- At least 1 year of Case Management or Utilization Review experience strongly preferred
- Candidates with a Home Health or insurance background may be considered with adequate acute care RN experience
- Strong critical thinking, organizational, and communication skills

Case Management certification (e.g., CCM or ACM) preferred but not required
