Setting: Primary Care Outpatient (Mon-Fri, no weekends)

Position Summary:

The Registered Nurse (RN) plays a critical role in delivering direct patient care, supporting care coordination, and advocating for the unique needs of our patients. This role requires a strong clinical foundation, cultural humility, and a deep commitment to health equity. The RN will work closely with a multidisciplinary team to assess, plan, and implement high-quality care in a fast-paced, resource-conscious environment.


Key Responsibilities:

Direct Patient Care:

  • Conduct comprehensive nursing assessments and document findings in the EHR
  • Administer medications, vaccines, and treatments according to clinical protocols
  • Provide wound care, chronic disease management, and preventive services
  • Assist providers during procedures and exams


Patient Education & Advocacy:

  • Deliver education on health conditions, medications, and self-management strategies
  • Partner with social workers, case managers, and community resources to address social determinants of health
  • Serve as a patient advocate, promoting informed decision-making and respect for cultural values


Care Coordination & Team Support:

  • Nurses can have refill protocol privileges, allowing them to authorize prescription refills under specific, pre-defined conditions set by a physician
  • Coordinate referrals, diagnostic testing, and follow-up care
  • Monitor patient progress and collaborate with providers to adjust care plans as needed
  • Participate in quality improvement initiatives and team-based care huddles
  • Assist physicians with procedures (i.e. joint injections)


Qualifications:

  • Current RN license in Maryland
  • Associate’s or Bachelor’s degree in Nursing (BSN preferred)
  • BLS certification required; ACLS preferred
  • At least 1 year of clinical experience (community health, primary care, or acute care experience preferred)
  • Bilingual or multilingual candidates strongly encouraged to apply
  • Prior experience working with underserved populations is highly desirable
  • New grads are welcome
