Position: Hospice Admissions Nurse (RN)

Location: Columbus, OH

Schedule: Full-time, M-F 8:30-4:30pm with every other weekend On-Call Rotation

Travel: Required – *350 Gas Card Provided + Auto Allowance + mileage reimbursement


About the Role:

We are seeking a compassionate and skilled Hospice RN Case Manager to provide patient-centered end-of-life care in homes, assisted living facilities, and skilled nursing settings. This role focuses on symptom management, emotional support, and ensuring comfort for patients and their families. As a key member of the interdisciplinary care team, you will coordinate services that uphold the highest quality of hospice care.


Key Responsibilities:

  • Conduct comprehensive patient and family assessments to develop individualized care plans.
  • Monitor patient conditions, manage symptoms, and provide direct nursing care in home and facility settings.
  • Administer medications and treatments in alignment with physician-directed plans of care.
  • Collaborate with an interdisciplinary team, including physicians, social workers, chaplains, and aides, to provide holistic care.
  • Educate families and caregivers on symptom management, medication administration, and end-of-life care practices.
  • Maintain thorough and timely documentation of patient status and interventions.
  • Communicate effectively with physicians and care teams regarding changes in patient conditions.
  • Participate in on-call rotations to ensure 24/7 access to hospice support.


Qualifications:

  • Registered Nurse (RN) with an active license in the state.
  • Minimum of one (1) year of experience in hospice, home health, or a related field preferred.
  • Strong clinical assessment and critical thinking skills.
  • Excellent communication and interpersonal skills for working with patients, families, and care teams.
  • Ability to travel locally within a designated radius.
