About the Role:

We’re seeking a dedicated Nurse Practitioner to join our Salem, Oregon care team. You’ll provide a blend of clinic-based and in-home services within a 30-mile radius, working closely with high-risk patients in need of continuity and proactive medical attention. This is a full-time, weekday role focused on relationship-building, preventive health, and effective chronic disease management.


What You'll Do:

  • Conduct comprehensive patient assessments, both in clinic and home-based settings
  • Perform annual wellness visits, risk adjustment coding, and medical reconciliation
  • Monitor and manage chronic conditions through evidence-based protocols
  • Collaborate with interdisciplinary team members including social workers and care coordinators
  • Support patient transitions following hospital discharges and develop care plans to reduce readmission risk
  • Utilize EMR systems for timely, accurate documentation (72-hour turnaround expected)
  • Participate in care team meetings to enhance holistic treatment approaches
  • Provide high-touch, patient-centric education on diagnoses, medications, and long-term care planning
  • Rotate coverage with other NPs as needed to ensure consistent access for patients
  • Maintain confidentiality in accordance with HIPAA and uphold documentation best practices
  • Expected daily caseload:
  • Clinic: ~10–12 patients
  • In-Home: ~5–6 patients


Qualifications:

  • Current and unrestricted Nurse Practitioner license in Oregon
  • Board certification (FNP, AGNP, or related) through ANCC or AANP
  • Minimum of 2 years’ experience in adult/geriatric care, primary care, internal medicine, or related field
  • Comfortable working with complex patients in home and outpatient environments
  • Familiarity with Medicare populations, risk-based care, and HCC documentation
  • Active DEA registration or willingness to obtain
  • Valid Oregon driver’s license
  • CPR/BLS certification
  • Home health or transitional care experience is a plus
