POSITION OVERVIEW
CG Oncology is seeking an accomplished, strategic, and execution-focused Vice President, Market Access to build and lead the U.S. Market Access organization supporting the anticipated launch of cretostimogene grenadenorepvec in non-muscle invasive bladder cancer (NMIBC). Reporting to the Chief Commercial Officer and serving as a key member of the Commercial Leadership Team, this executive will have enterprise responsibility for payer strategy, pricing and contracting, reimbursement and coding, trade and distribution, patient services, field reimbursement, provider economics, and access operations.
This is a significant build-and-launch leadership role. The successful candidate will establish the capabilities and infrastructure required for CG Oncology's first commercial launch, including the coding, coverage, reimbursement, distribution, provider-economic, and patient-access foundation necessary for a provider-administered therapy in a predominantly Medicare population.
The role requires particular expertise navigating buy-and-bill reimbursement and the economics of treatment across community urology practices, ambulatory surgery centers (ASCs), and hospital outpatient departments. This leader will work closely with Sales, Marketing, Commercial Operations, Medical Affairs, IVES, Finance, Regulatory Affairs, Legal, Clinical Development, Patient Advocacy, Quality, Supply Chain, and executive leadership to ensure access considerations are fully integrated into launch strategy and execution.
CORE RESPONSIBILITIES
- Lead the integrated U.S. Market Access strategy for cretostimogene across BCG-unresponsive high-risk NMIBC and future indications, connecting the product's clinical profile with payer requirements, patient needs, and provider economics.
- Own coding, coverage, and reimbursement strategy from pre-launch through permanent coding and mature reimbursement, including HCPCS/J-code planning, transitional reimbursement where applicable, administration coding, Medicare Part B, OPPS, ASC payment environments, MAC engagement, commercial payers, Medicaid, and organized customers.
- Lead launch pricing strategy, contracting architecture, ASP-based reimbursement planning, gross-to-net inputs, 340B strategy, government pricing coordination, and governance for discounts, concessions, and access investments.
- Design and implement the commercial distribution and channel model for a temperature-controlled biologic, including specialty distribution, logistics, inventory management, order-to-cash, chargebacks, data flows, and channel partner performance.
- Build patient services and access programs that reduce time to therapy and simplify treatment access, including hub services, benefits verification, prior authorization, appeals, financial assistance, reimbursement support, enrollment infrastructure, digital tools, and provider education.
- Develop provider-economic strategies and tools for community urology practices, ASCs, and hospital outpatient departments, translating acquisition, administration, reimbursement, inventory, and cash-flow considerations into practical customer support.
- Build and lead the Field Reimbursement organization, including organizational design, deployment strategy, training, compliance, performance management, and rapid resolution of account-level reimbursement and access barriers.
- Partner with Medical Affairs, HEOR, and Clinical Development on evidence generation supporting payer and provider value, including treatment pathways, healthcare resource utilization, cost offsets, downstream utilization, and potential avoidance or delay of radical cystectomy.
- Lead enterprise launch readiness through an integrated Market Access plan with milestones, dependencies, owners, risks, budgets, KPIs, and escalation mechanisms; provide executive leadership with timely visibility into access risks and required tradeoffs.
- Build a scalable Market Access organization, recruit and develop exceptional talent, establish clear accountability and decision rights, and effectively manage agencies, consultants, vendors, and strategic partners.

PROFESSIONAL QUALIFICATIONS
- Bachelor's degree required; MBA, PharmD, MPH, or related advanced degree preferred.
- 15+ years of progressive biopharmaceutical experience with significant leadership responsibility in Market Access.
- Demonstrated success developing and executing Market Access strategy for oncology, urology, or other complex specialty products.
- Significant U.S. product launch experience, ideally involving a first-in-class or differentiated specialty therapy.
- Direct experience with provider-administered, buy-and-bill products, including ASP mechanics, HCPCS coding, Medicare Part B reimbursement, and hospital outpatient reimbursement.
- Strong understanding of commercial payer, Medicare, Medicaid, IDN, and provider access dynamics.
- Deep expertise across pricing, contracting, payer strategy, reimbursement, trade and distribution, patient services, and field reimbursementDemonstrated ability to translate reimbursement and provider economics into actionable commercial strategy.
- Experience building teams, infrastructure, and organizational capabilities in a pre-commercial, emerging biotechnology, or first-launch environment.
- Proven ability to operate as an enterprise leader, influence senior executives and cross-functional stakeholders, and recruit, develop, and lead high-performing teams.
Preferred Experience
- U.S. launch experience with a specialty oncology or urology product used in community practices and/or ASC settings.
- Experience launching products into a predominantly Medicare population.
- Deep working knowledge of 340B dynamics relevant to provider-administered products.
- Experience establishing permanent product coding and navigating transitional reimbursement periods following launch.
- Familiarity with intravesical, intratumoral, or other locally administered therapies and associated site-of-care considerations.
- Experience with biologic, cell, gene, viral, or similarly complex therapies requiring specialized handling and distribution.
- Experience designing and implementing specialty distribution and patient services models from the ground up.
- Track record of successful cross-functional leadership during complex product launches and establishing scalable Market Access capabilities for future indications and portfolio expansion.
