Position Overview
The Primary Care Physician will provide comprehensive outpatient care for patients across the lifespan while collaborating closely with an interdisciplinary care team to ensure high-quality, culturally responsive care.
Physicians will have a primary “home clinic” location in either Lake Forest or San Clemente, with flexibility to support nearby clinic sites as needed.
Clinical Responsibilities
- Provide full-spectrum outpatient primary care services
- Diagnose and manage acute and chronic medical conditions
- Coordinate specialty referrals and continuity of care
- Deliver evidence-based, patient-centered treatment plans
- Participate in quality improvement and collaborative care initiatives
- Support and mentor clinical team members as appropriate
- Educate patients on prevention, wellness, and chronic disease management
Practice Environment
- Average patient volume: approximately 18 patients per day
- Scheduled template supports manageable workflow
- Dedicated support staff including bilingual Medical Assistants
- Integrated referral and case management infrastructure
- Predominantly Spanish-speaking patient population
- Evening hours may be required occasionally, though schedules are designed to support work-life balance
Qualifications
Required
- MD or DO from an accredited medical school
- Board Certified or Board Eligible in:
- Family Medicine (preferred)
- Internal Medicine
- Active California medical license or ability to obtain
- DEA certification or eligibility
Preferred
- Community Health Center or FQHC experience
- Spanish-speaking skills strongly valued
- Passion for underserved and multicultural patient populations
- Adaptability and strong team collaboration skills
- Comfort working in a mission-driven environment
