POSITION SUMMARY

The Clinician (NP/PA) delivers outpatient primary care services in collaboration with a supervising physician. This role is responsible for evaluating, diagnosing, and managing a wide range of patient conditions while promoting preventive care and long-term health outcomes.

ESSENTIAL DUTIES AND RESPONSIBILITIES

  • Conduct patient evaluations, including medical histories and physical examinations
  • Diagnose and manage acute and chronic conditions
  • Develop and implement individualized treatment plans
  • Order and interpret diagnostic tests such as labs and imaging
  • Prescribe medications and therapeutic interventions as appropriate
  • Provide preventive care, screenings, and health education
  • Coordinate care with specialists and internal support services
  • Address behavioral, social, and environmental factors impacting patient health
  • Maintain accurate, timely, and compliant medical documentation
  • Participate in care team collaboration, including daily huddles
  • Contribute to quality improvement and patient care initiatives
  • Provide guidance and support to clinical staff as needed
  • Participate in on-call or after-hours support as required

QUALIFICATIONS

  • Graduate of an accredited Nurse Practitioner or Physician Assistant program
  • Active, unrestricted California NP or PA license
  • Current BLS/CPR certification
  • DEA registration (or ability to obtain)
  • Experience in primary care or community health preferred
  • Strong clinical judgment and diagnostic skills
  • Excellent communication and interpersonal abilities
  • Ability to work effectively in a collaborative, team-based environment
