Position Summary

The Nurse Practitioner or Physician Assistant provides comprehensive, patient-centered outpatient primary care to individuals across the lifespan. This role focuses on delivering high-quality care in a collaborative, team-based environment while addressing both medical and social determinants of health. The position offers the opportunity to make a meaningful impact in an underserved community, with eligibility for loan repayment programs.

Key Responsibilities

  • Provide full-scope outpatient primary care, including preventive, chronic, and acute care
  • Diagnose and treat a wide range of conditions across pediatric, adult, and geriatric populations
  • Prescribe medications and develop treatment plans in collaboration with supervising physician
  • Order and interpret laboratory tests, imaging, and other diagnostics
  • Coordinate referrals to specialty and ancillary services
  • Maintain accurate and timely electronic health record (EHR) documentation (within 24 hours)
  • Support continuity of care and efficient clinic operations
  • Collaborate within a multidisciplinary, team-based care model
  • Participate in quality improvement initiatives and community health prog
  • Deliver culturally competent, compassionate care to a diverse patient population

Qualifications

  • Master’s degree from an accredited Physician Assistant program or Nurse Practitioner program
  • Active, unrestricted Texas Physician Assistant or Nurse Practitioner license
  • Board certification
  • Current DEA registration
  • Basic Life Support (BLS) certification (AHA)
  • Experience in community health, FQHC, or underserved settings preferred
  • Bilingual in Spanish strongly preferred
