Family Medicine Physician – Community Health Center (FQHC), Bay Area, CA

Position Summary

The Family Medicine Physician provides comprehensive, patient-centered primary care to patients across the lifespan in a community health setting. This role focuses on delivering high-quality care in a team-based, mission-driven environment that addresses both medical and social determinants of health. Key Responsibilities

  • Provide full-scope outpatient primary care, including preventive, chronic, and acute care
  • Diagnose and treat a wide range of conditions across pediatrics, adults, and geriatrics
  • Collaborate within a team-based care model including medical assistants, behavioral health providers, and care coordinators
  • Participate in care coordination and population health management
  • Maintain accurate and timely electronic health record (EHR) documentation
  • Support and engage in quality improvement initiatives
  • Deliver culturally competent care to a diverse patient population

Qualifications

  • MD or DO degree from an accredited institution
  • Board certified or board eligible in Family Medicine (or Internal Medicine)
  • Active or eligible for California medical license
  • Experience in community health, FQHC, or underserved settings preferred
  • Bilingual in Spanish strongly preferred

  • Support staff includes two medical assistants per provider, with approximately 80% Spanish-speaking
  • Productivity expectation of approximately 10 completed appointments per 4-hour session
