Director of Case Management

Full Time | Days | No Weekends | 7 to 10 Hour Shifts

Job Summary

The Director of Case Management provides leadership and oversight for all hospital case management and care coordination operations. This role ensures compliance with regulatory standards, organizational policies, and best practices while driving high quality patient outcomes, efficient resource utilization, and effective discharge planning. The Director collaborates closely with clinical, financial, and administrative leadership to ensure seamless transitions of care and strong interdisciplinary coordination.

This role requires an experienced healthcare leader with strong communication, change management, and team building skills. The Director is responsible for aligning case management operations with organizational goals while fostering a culture of accountability, performance excellence, and patient centered care.

Key Responsibilities

  • Lead and oversee all case management operations including staffing models, workflow design, onboarding, and competency development
  • Ensure effective care coordination and discharge planning processes with accurate, timely documentation
  • Manage department productivity, quality, and performance metrics
  • Allocate resources to meet patient volume, acuity, and operational needs
  • Collaborate with finance and clinical leadership to achieve organizational goals
  • Monitor data and implement continuous performance improvement initiatives
  • Ensure compliance with policies, regulatory requirements, and standards of practice
  • Build strong relationships with medical staff, nursing leadership, ancillary services, compliance, and legal teams
  • Serve as a subject matter expert in hospital case management and care coordination

Qualifications

  • Bachelor’s degree required
  • Master’s degree in Nursing, Health Administration, or Business Administration preferred
  • Active RN, LCSW, or LMSW license required
  • Minimum of 3 years recent acute care hospital case management experience
  • Minimum of 2 years in case management leadership

Strong candidates will bring experience from large, fast paced hospital environments and demonstrate proven leadership, operational expertise, and the ability to lead complex teams.

Compensation and Benefits

  • Salary range: $117,423 to $155,000 based on experience
  • Incentive bonus up to 17.5 percent
  • Sign on bonus: $10,000
  • Relocation assistance available on a case by case basis
