The Care Coordination Director plays a key role in delivering high-quality, seamless care to our clients.
Role:
As the Care Coordination Director, you will lead a dedicated team to guarantee that each client's home care plan meets their needs, family expectations, and regulatory requirements. This includes maintaining continuous communication and coordination with referral sources, family members, clients, healthcare providers, and intake teams. You'll oversee efficient scheduling of care services, while ensuring clients and their families receive exceptional service and attention.
Responsibilities:
**Client Assessment and Care Planning:**
- Ensure the accurate development and ongoing maintenance of reporting processes for the department
- Provide ongoing coaching and support to coordinators, ensuring adherence to care plans and quality standards.
**Scheduling and Coordination:**
- Oversee caregiver visit scheduling to guarantee full coverage of clients' needs.
- Implement processes to ensure smooth coordination with clients and caregivers during schedule changes, emergencies, and special requests.
- Maintain meaningful communication with clients, caregivers, and healthcare professionals to facilitate seamless care coordination.
- Create an environment where your department is the primary point of contact for clients and their families, addressing inquiries, concerns, and feedback promptly and empathetically.
Requirements:
- Proven experience in healthcare/home care coordination, customer service, or staffing as a department leader or supervisor.
- Advanced knowledge of HHAeXchange.
- Strong communication skills with employees, patients, and families.
- Problem-solving skills: ability to assess challenges, collect data, and implement solutions.
- Minimum 2 years of experience leading within a licensed home care services agency (LHCSA).
- Ability to build trust and foster relationships with Coordinators and HHAs.
- Highly motivated and thrives in a fast-paced environment.
