Nurse Practitioner – Home-Based Care (Philadelphia, PA)

Our client is seeking a compassionate and skilled Nurse Practitioner to join their mission-driven clinical team, providing high-quality, patient-centered care throughout the Philadelphia area. Key Responsibilities

• Perform in-home health assessments, wellness checks, and preventative care services.
• Develop and manage personalized care plans, with a focus on prevention, chronic disease management, and long-term health outcomes.
• Conduct screenings, geriatric evaluations, and wound care treatments (wound care experience is a strong plus).
• Educate patients and caregivers on wellness, healthy living practices, and disease prevention.
• Collaborate closely with an interdisciplinary care team and maintain accurate, timely medical documentation.
• Utilize clinical judgment to operate independently while leveraging support from the broader clinical team.

Qualifications

• Active Nurse Practitioner (NP) certification and current state licensure (required).
• Prior experience in home-based care, primary care, or with geriatric populations (preferred).
• Wound care experience and/or certification is highly valued.
• Strong clinical assessment, communication, and problem-solving skills.
• Ability to work independently while maintaining collaboration with remote team members.
• Reliable transportation and willingness to travel to patient homes within the Philadelphia region.
