Job Title: Telehealth Nurse Practitioner – Geriatric Care (Spanish-Speaking)

Location: Remote (Serving Patients in New York)

Schedule: Full-Time, 40 Hours per Week | Flexible Hours Between 8:00 AM – 7:00 PM

About the Role:

We are seeking 2–3 experienced Nurse Practitioners to join our growing telehealth team and provide high-quality, compassionate care to our geriatric patients in a home setting. This is an immediate hire opportunity for providers who thrive in a fast-paced environment, value patient-centered care, and are fluent in Spanish.

Key Responsibilities:

• Conduct telehealth visits with geriatric patients in their home setting (15–30 minutes per visit)
• Target an average of 20 patient visits per day
• Perform thorough patient assessments, diagnose, and manage chronic and acute conditions
• Prescribe medications as appropriate, in compliance with state and federal regulations
• Document all patient interactions in Enable Healthcare EHR
• Coordinate care with other healthcare providers and services as needed
• Ensure compliance with Medicaid, Medicare, and other payer requirements

Qualifications:

• Current Nurse Practitioner license in New York (required)
• Valid DEA registration (required)
• Credentialed with Medicaid and Medicare (required)
• Fluent in Spanish (required)
• Minimum 1–2 years’ experience as a Nurse Practitioner, preferably in geriatric or primary care
• Strong telehealth and technology skills, including comfort working with EHR systems
• Exceptional communication and time-management abilities
