We are representing a highly rated skilled nursing and rehabilitation facility located in Oakdale, NY, in search of an experienced MDS Coordinator (Registered Nurse). Position Summary:

The MDS Coordinator is responsible for the accurate and timely completion of Minimum Data Set (MDS) assessments and care plans in accordance with federal and state guidelines. This role plays a vital part in optimizing resident outcomes and ensuring appropriate reimbursement.


Key Responsibilities:

  • Coordinate and manage the MDS/RAI process across all residents
  • Ensure timely submission of MDS assessments and related documentation
  • Collaborate with clinical and interdisciplinary teams to develop individualized care plans
  • Audit clinical documentation to ensure accuracy and compliance with regulatory requirements
  • Educate nursing staff on proper documentation to support care and reimbursement

Qualifications:

  • Current RN license in the State of New York
  • Experience with MDS 3.0 and RAI process required
  • Familiarity with PDPM and Medicaid case mix models strongly preferred
  • Strong attention to detail, organizational skills, and ability to work independently
  • Proficiency with electronic medical record systems and MDS software
