Position Summary:
The MDS Coordinator is responsible for the accurate and timely completion of Minimum Data Set (MDS) assessments and care plans in accordance with federal and state guidelines. This role plays a vital part in optimizing resident outcomes and ensuring appropriate reimbursement.
Key Responsibilities:
- Coordinate and manage the MDS/RAI process across all residents
- Ensure timely submission of MDS assessments and related documentation
- Collaborate with clinical and interdisciplinary teams to develop individualized care plans
- Audit clinical documentation to ensure accuracy and compliance with regulatory requirements
- Educate nursing staff on proper documentation to support care and reimbursement
Qualifications:
- Current RN license in the State of New York
- Experience with MDS 3.0 and RAI process required
- Familiarity with PDPM and Medicaid case mix models strongly preferred
- Strong attention to detail, organizational skills, and ability to work independently
- Proficiency with electronic medical record systems and MDS software
