Job Title: Registered Nurse (RN) – Home Care: Client Assessments & Caregiver Support

Position Summary:
We are seeking a compassionate and detail-oriented Registered Nurse (RN) to join our team. This hybrid role involves conducting in-home client assessments, developing care plans, and providing office-based support to caregivers to ensure consistent, high-quality service delivery.
Key Responsibilities:
Client Assessments & Care Planning
- Conduct in-home initial assessments for new clients to determine care needs, environment, and caregiver compatibility.
- Develop individualized care plans in collaboration with clients and families, aligned with agency capabilities and client goals.
- Perform periodic client reassessments and update care plans as needed based on changes in condition or client feedback.
- Document findings and maintain accurate client records in compliance with regulatory standards and agency policy.
Caregiver Oversight & Support
- Serve as a clinical resource and point of contact for field caregivers regarding client care concerns, care plan clarification, and problem-solving.
- Provide guidance on best practices in personal care, safety, and infection prevention.
- Review caregiver documentation for accuracy and completeness; follow up on incidents, changes in condition, or client complaints.
- Participate in caregiver onboarding by reviewing clinical scenarios and assisting with competency verification.
Office-Based Collaboration
- Communicate regularly with the scheduling, intake, and administrative teams to ensure care continuity and staffing alignment.
- Participate in staff meetings, quality assurance reviews, and compliance audits as needed.
- Assist with training sessions, in-services, or caregiver education programs.
Qualifications:
- Active RN license in the state of Georgia in good standing.
- Minimum of 1–2 years of clinical experience; home care or geriatric care experience strongly preferred.
- Excellent clinical judgment, organizational skills, and attention to detail.
- Strong interpersonal and communication skills; ability to engage empathetically with clients and caregivers.
- Ability to work independently in both the field and office settings.
- Proficiency in using electronic health records (EHR) and Microsoft Office; experience with home care software (e.g., ClearCare, AlayaCare) is a plus.
- Valid driver's license and reliable transportation.
