RN, Team Manager - Palliative Care

Employment Type: Full-Time | Leadership

Location: Palm Springs, CA

Compensation: $50.00 – $61.00 an hour


The RN Team Manager (Palliative Care) is primarily responsible for supervising the Palliative Care Interdisciplinary Group (IDG). This leadership role focuses on managing, coordinating, and facilitating high-quality palliative care services for members, ensuring regulatory compliance, operational efficiency, and optimized patient outcomes.


Responsibilities:

  • Direct and supervise professional and administrative personnel within the Interdisciplinary Group. Produce equitable team schedules and patient allocations based on case acuity and geographic location to meet departmental productivity expectations
  • Perform concurrent clinical reviews of patient plans of care to identify required clinical interventions. Oversee IDG care coordination to optimize appropriate resource utilization (e.g., driving utilization toward Immediate Care vs. the Emergency Room)
  • Track and collect data for the Palliative Care Referral Log and monthly encounter metrics for leadership presentations. Partner with the Quality Manager to audit and enhance dashboards tracking 5-Star Measures, HCC coding gaps, and "Care of the Adult" metrics
  • Facilitate strategic discharge planning and seamless program transitions for members who no longer meet the clinical eligibility criteria for Palliative Care services
  • Participate in executive leadership meetings, Quality Assessment Performance Improvement (QAPI) initiatives, and the administrative on-call rotation. Manage cost-containment strategies and continuous operational audits
  • Cultivate a collaborative, solutions-oriented, and positive environment within the team. Maintain clear lines of communication regarding availability and provide ongoing professional development and performance insights to staff

Requirements:

  • Current, valid California Registered Nurse (RN) License
  • At least 2 years of clinical supervisory or management experience, plus recent hands-on nursing experience within the last 3 years
  • Deep understanding of Medicare Conditions of Participation (CoPs), California Standards of Hospice Care, and other federal and state regulations governing hospice and palliative care
  • High proficiency in computer applications, medical databases, and electronic dashboard tracking
  • Strong multitasking, problem-solving, and leadership capabilities. Valid California driver’s license with proof of auto insurance required


Physical Demands & Work Environment:

  • Desk and field-balanced role involving sitting (60%), standing (20%), and walking (30%).
  • Requires frequent reaching above the head (10%) and occasional lifting of equipment/supplies up to 10 lbs.
  • Highly technical workspace utilizing computers, cell phones, and tablets for more than 6 hours per day
