Advanced Practice Provider: Physician Assistant/ Nurse Practitioner

Compensation: $150K per year

Job Type: Full time, Monday - Friday. Must be able to travel between 2 locations in Brooklyn.

Our client, a healthcare organization serving clinics and shelters, is seeking a full-time Physician Assistant or Nurse Practitioner to work across two Brooklyn locations.

Duties and Responsibilities:

• Provide medical care to pediatric, adolescent, and adult patients.
• Take patient history, assess needs, and make diagnoses.
• Develop and implement treatment plans using diagnostic data.
• Perform physical exams, including well-care and gynecological exams.
• Prescribe medications and educate patients on usage.
• Complete medical forms and documentation.
• Manage referrals and coordinate follow-up care.
• Assist with immunizations and phlebotomy as needed.
• Collaborate with physicians and specialists for consultations and referrals.
• Meet regularly with a supervising physician for guidance and quality care.
• Participate in peer review and quality improvement processes.
• Provide health education on preventive care, chronic disease management, and wellness topics.
• Advocate for patients and engage in outreach efforts.
• Ensure compliance with HIPAA and organizational policies.
• Supervise medical assistants at satellite sites.
• Train medical assistants to improve patient care and clinic efficiency.
• Participate in case conferences to discuss patient care.
• Assist in developing medical protocols and forms.
• Contribute to research initiatives in collaboration with leadership.
• Recommend improvements to enhance patient care and clinic operations.

Qualifications:

• Minimum of 2 years of experience in Family Medicine required.
• Must have independent access to the Medicaid system.
• Active and in-good-standing PA or NP license in New York State.
