Advanced Practice Provider: Physician Assistant/ Nurse Practitioner
Compensation: $150K per year
Job Type: Full time, Monday - Friday. Must be able to travel between 2 locations in Brooklyn.
Our client, a healthcare organization serving clinics and shelters, is seeking a full-time Physician Assistant or Nurse Practitioner to work across two Brooklyn locations.
Duties and Responsibilities:
- Provide medical care to pediatric, adolescent, and adult patients.
- Take patient history, assess needs, and make diagnoses.
- Develop and implement treatment plans using diagnostic data.
- Perform physical exams, including well-care and gynecological exams.
- Prescribe medications and educate patients on usage.
- Complete medical forms and documentation.
- Manage referrals and coordinate follow-up care.
- Assist with immunizations and phlebotomy as needed.
- Collaborate with physicians and specialists for consultations and referrals.
- Meet regularly with a supervising physician for guidance and quality care.
- Participate in peer review and quality improvement processes.
- Provide health education on preventive care, chronic disease management, and wellness topics.
- Advocate for patients and engage in outreach efforts.
- Ensure compliance with HIPAA and organizational policies.
- Supervise medical assistants at satellite sites.
- Train medical assistants to improve patient care and clinic efficiency.
- Participate in case conferences to discuss patient care.
- Assist in developing medical protocols and forms.
- Contribute to research initiatives in collaboration with leadership.
- Recommend improvements to enhance patient care and clinic operations.

Qualifications:
- Minimum of 2 years of experience in Family Medicine required.
- Must have independent access to the Medicaid system.
- Active and in-good-standing PA or NP license in New York State.
