Primary Care Physician

Compensation: $170K-$190K pr/yr

Job Type: Full Time, 4-5 days per week. 40 hours.

Our client a well established community health organization in Staten Island, NY is seeking a dedicated and skilled Primary Care Physician to add to there team.

Responsibilities:

• Delivers high-quality medical care in alignment with professional standards and organizational policies.
• Reviews patient medical histories during initial visits and maintains accurate documentation in the electronic medical record system.
• Manages and coordinates medical care for assigned patients.
• Evaluates walk-in patients, incoming calls, and referrals, ensuring appropriate medical care coordination.
• Engages in regular clinical and administrative supervision.
• Collaborates with the healthcare team to optimize patient care and coordination, participating in daily huddles and team meetings.
• Supports patient care initiatives related to specialized programs, including care planning for high-risk patients.
• Contributes to ongoing comprehensive care planning and review.
• Provides onsite medical interventions as needed.
• Facilitates patient referrals to in-network or external healthcare providers for necessary services.
• Promotes cultural sensitivity in patient care and fosters awareness among staff.
• Carries out tasks and initiatives as assigned by leadership.
• Ensures timely follow-up on abnormal test results or physical findings.

Job Requirements:

• Holds a Medical Degree (MD) from an accredited school
• Currently licensed and registered as a physician with New York State
• Board Certified or Board Eligible in a recognized medical specialty
• Medicaid and Medicare certified
• BLS Certified
