HCP Talent is collaborating with a Long-Term Care/Rehabilitation (280 beds) in Arverne, NY 11692 that is seeking a Social Work Director (MSW) for a full-time Day shift (7a-3p) M-F position.

SW Duties and Responsibilities:

• Ensure that all Social Work documentation is completed in accordance with facility policy and procedure as applicable
• Coordination of Interdisciplinary Discharge Planning (Knowledge of community resources
• Demonstrate compliance with government and accrediting agency standards and regulations pertaining to Social Work
• Interface with facility departments and outside agencies in order to provide a complete plan of care and to facilitate services.
• Liaison between family and other interdisciplinary team members

Qualifications & Requirements:

• Masters in Social Work
• Experience as a Director of Social Work required
• 1+ year of Long-Term Care Experience is Required
