What You’ll Do

• Conduct in-home assessments and develop individualized care plans (OASIS documentation)
• Provide direct skilled nursing care, including wound care, IV therapy, monitoring, and education
• Coordinate with physicians, therapists, aides, and family members to ensure holistic care
• Supervise and guide LPNs and Home Health Aides to support care quality
• Monitor and report changes in condition, update care plans, and facilitate discharge planning

What You’ll Need to Succeed

• Active RN license in Pennsylvania
• 1+ years of nursing experience in home health, hospital, or community care
• Working knowledge of Medicare/Medicaid guidelines and home health regulations
• Strong critical thinking, organization, and communication skills
• Valid driver’s license, reliable transportation, and CPR certification
