Nurse Practitioner or Physician Assistant (FP or IM) Post Acute Care –  Mohawk Valley region, NY

Position Highlights:

• Full-time or Part-Time, Monday–Friday daytime schedule
• Manage 15–22 patients per day in post-acute care settings
• No night call responsibilities
• Collaborate with interdisciplinary care teams and community partners including hospice, palliative care, and home health

Job Responsibilities:

• Provide high-quality, cost-effective primary care in post-acute care communities
• Ensure accurate and timely documentation in EMR (Gehrimed) per Medicare guidelines
• Support safe patient transitions and care coordination
• Participate in CMS-aligned quality initiatives
• Collaborate with other advanced practice providers and physicians

Compensation & Benefits:

• Estimated annual compensation: $114,000 – $136,000 (productivity-based, no cap)
