Our incredible team is expanding, and we're searching for an enthusiastic Intake RN to join us in empowering frail and disabled individuals to live independently in their homes for as long as possible. Imagine the joy of guiding new members and their families through a seamless transition to managed long-term care!
What You'll Do (and Love!):
- Be a Community Champion! Conduct exciting outreach activities, assessing eligibility and ensuring the safety of potential members.
- Team Up for Success! Collaborate closely with our Intake/Outreach and clinical teams to guarantee new members receive the perfect services.
- Connect and Communicate! Engage with potential members, their families, referral sources, and physicians, building strong, trusting relationships.
What You'll Bring (and We'll Celebrate!):
- A current, shining New York State Registered Nurse License.
- An Associate's Degree in Nursing (required) – a Bachelor of Science in Nursing (BSN) is a fantastic bonus!
- One (1) year of acute care experience under your belt.
- Essential UAS-NY experience – you're a pro!
- Home care and management experience? That's a huge plus!
- Proficiency in MS Office – you're tech-savvy!
- Strong, vibrant oral, written, and interpersonal communication skills – you're a natural connector!
- Bilingual in Chinese or Spanish? You'll be a superstar!
