Job Description:
Our client is seeking a dedicated and highly proficient Clinical Pharmacy Specialist to focus on solid organ transplantation, with a primary emphasis on kidney transplant patients. This is a direct patient care role that encompasses the entire continuum of care, from initial patient selection through long-term post-discharge follow-up. You will play a pivotal role in optimizing medication regimens, particularly immunosuppressive drugs, to ensure long-term allograft preservation. Your responsibilities will also include managing treatments for co-morbidities and providing comprehensive medication education to patients and their families during critical transitions of care.
A key part of the position involves participating in multidisciplinary rounds on the inpatient service, where you will provide your expert clinical judgment. You will facilitate the appropriate selection of treatments for cases of acute or chronic allograft injury or rejection, leveraging established protocols. The role also includes teaching and mentorship opportunities, as you will serve as a preceptor for pharmacy students and residents, fostering the next generation of pharmacy leaders. Beyond patient care, there are ample chances to contribute to longitudinal projects, including research, medication use evaluations, and process improvement initiatives.
