.We are looking for a compassionate and detail-oriented Care Manager to oversee client care plans and ensure high-quality services for individuals in need. The ideal candidate will have strong organizational and communication skills to collaborate with caregivers, clients, and families to optimize care solutions.

Key Responsibilities:

• Develop and oversee individualized care plans for clients.
• Coordinate with caregivers, healthcare professionals, and families.
• Monitor client progress and adjust care plans as needed.
• Ensure compliance with homecare policies and procedures.
• Advocate for clients' needs and provide emotional support.
• Maintain accurate documentation and case records.

Qualifications:

• BA or MA in Social Work, Healthcare Administration, or a related field.
• Previous experience in care management, social work, or homecare preferred.
• Excellent organizational and problem-solving skills.
• Strong communication and interpersonal abilities.
• Ability to multitask and work in a fast-paced environment.

Salary: $60K-$70K
