The Centralized Admissions professional will manage and coordinate all incoming referrals for Skilled Nursing Facilities, ensuring timely and appropriate admissions. This role requires reviewing clinical and financial documentation, verifying insurance coverage, communicating with hospitals and referral sources, and determining admission eligibility.

The ideal candidate understands the operational and financial aspects of SNF admissions, can work efficiently in a fast paced environment, and collaborates closely with facility teams to maximize census while maintaining compliance and strong referral relationships.

Key Responsibilities:

• Review incoming referrals and evaluate admission eligibility

• Assess clinical documentation and financial information

• Verify insurance coverage and benefits

• Communicate with hospitals, discharge planners, and referral sources

• Coordinate with facility teams to ensure smooth admissions

• Maintain compliance with regulatory and payer requirements

• Track referral activity and support census growth initiatives

• Build and maintain strong referral relationships

Qualifications:

• Prior Skilled Nursing Facility admissions experience required

• Strong knowledge of insurance verification and payer requirements

• Ability to review and interpret clinical documentation

• Excellent communication and relationship management skills

• Highly organized with strong attention to detail

• Ability to manage multiple referrals in a fast paced setting
