Title: Staff Pharmacist (Evenings)

Status: Full-Time

Schedule: Monday-Friday, 1pm-9:30pm (every other weekend)

Location: Pittsfield, MA

The Staff Pharmacist provides comprehensive pharmaceutical care to inpatients and outpatients by ensuring the safe, effective, and cost-efficient use of medications. This role collaborates with physicians, nurses, and other healthcare professionals to optimize patient outcomes, comply with regulatory standards, and support the hospital’s mission of delivering high-quality care.

Key Responsibilities:

• Review and interpret physician medication orders for appropriateness, accuracy, and potential drug interactions.
• Prepare, dispense, and verify medications in accordance with hospital policies, formulary guidelines, and state/federal regulations.
• Provide drug information and clinical consultation to healthcare providers and patients.
• Monitor patient therapy for efficacy, side effects, and adverse drug reactions; recommend adjustments as needed.
• Participate in multidisciplinary patient care rounds, antimicrobial stewardship, and clinical initiatives.
• Oversee and guide pharmacy technicians in medication preparation, distribution, and inventory management.
• Ensure compliance with USP <797>/<800> standards for sterile and hazardous drug compounding.
• Maintain accurate and complete patient medication records within the electronic health record (EHR) system.
• Support medication safety programs, quality improvement initiatives, and formulary management.
• Participate in continuing education and maintain current licensure.

Qualifications:

• Bachelor’s in Pharmacy or Doctor of Pharmacy (PharmD) from an accredited program
• Active Massachusetts Pharmacist license (MA PHARM)
• 1-2 years of hospital pharmacy experience required
• Knowledge of IV admixture and chemotherapy preparation helpful
• Must be able to work independently and supervise support personnel
