Social Worker (MSW) – Acute Care Setting 🌟

📍 Location: Fort Walton Beach, FL

🕒 Shift: Days with rotating weekends (8:00 AM – 4:30 PM, may vary slightly)

About the Role:

We are seeking an experienced Licensed Clinical Social Worker (LCSW) with a passion for patient advocacy and discharge planning to join a dynamic hospital team. This role supports both inpatient and outpatient emergency department settings through biopsychosocial assessments, crisis intervention, emotional support, and coordination of post-acute care resources.

As part of the care team, you will collaborate with physicians, nurses, case managers, and families to develop discharge plans and address barriers such as financial, emotional, and psychosocial needs. This position requires strong organizational skills, critical thinking, and the ability to provide compassionate support during difficult circumstances.

Key Responsibilities:

• Complete thorough patient assessments and develop discharge plans for high-risk patients.
• Provide counseling, education, advocacy, and crisis intervention for patients and families.
• Collaborate with the healthcare team to coordinate safe and effective transitions of care.
• Assist patients and families in navigating community resources, financial assistance, and referrals.
• Support patients and families during emotionally challenging situations such as terminal illness or end-of-life care.

Qualifications:

✔ Master’s in Social Work (MSW) required

✔ Active Florida LCSW license required

✔ At least 1 year of recent acute care Social Work experience (will also consider candidates with 5+ years of past hospital experience if not recent)

✔ Must have discharge planning experience

✔ Experience with PASSRs and ESPERs required

✔ LTAC or Hospice experience may be considered with prior hospital experience
