Your Role with the Company:

• Full-time position, Flexible schedule and work/life balance
• 60-90 min visits specializing in peri menopause, GI, regenerative and functional medicine
• Provider will see 3-5 patients/day, home visits, some clinic visits and telehealth
• Nurse Practitioner will build up to carry a patient panel of 225 patients

Background Profile:

• FNP / Family Nurse Practitioner, APRN, Adult Gerontology Nurse Practitioner
• License and Independent practice authority in Illinois
• 2+ years of Hormone experience required, functional medicine experience desired
